Can a direct-vent insert go into an existing masonry fireplace?

Usually yes, but the chimney decides the details. A sealed insert needs a coaxial liner run from the unit to the top. That means two pipes, intake and exhaust, inside the old flue. Prospect Heights has a lot of clay tile liners from the 1920s through the 1950s. Some are unlined or barely lined. Shifted or cracked tiles narrow the usable space, so the liner diameter the manufacturer requires may not pass. We measure the smoke chamber and the tile throat before anything is ordered.

Check whether the furnace or water heater shares that chimney. Many older homes here vent appliances into the same masonry stack. If a high-efficiency furnace was installed later, the flue may now be orphaned and oversized for the water heater alone. Dropping a liner into that chimney changes the picture again. That gets sorted during the install, not after.

Prefab fireplaces in wood-framed chases are a different job. Clearances to framing are fixed by the listing, and the chase cover and firestops get rebuilt as part of it.

Gas supply matters too. Existing pipe already feeding a furnace, range, and dryer may not carry another load without resizing.
